- 1). Calculate the total correct amount of sales tax due using your sales receipts.
- 2). Review your original tax return to see where you made mistakes.
- 3). Place the new copy of ST-101 next to the original tax return to note changes that have to be made.
- 4). Fill out the new copy of ST-101. Begin by adding the information that has not changed. Then add the items that are changing.
- 5). Calculate the new tax due on the form.
- 6). Write "Amended Form" across the top of ST-10. If this is your final form for the year, write "Final Amended Form" instead.
- 7). Attach a copy of your original return, a copy of your Certificate of Authority and a check to the amended return.
- 8). Make a copy of the items for your records.
- 9). Send the form and attachments to your state's department of taxation and finance.
